Consent to the processing of personal data

In accordance with the Law of July 02, 2019 No. ZRU-547 “On Personal Data”, by my will and in my interest I express to JV ANDIJANPETRO LLC, located at the address: 101990, Republic of Uzbekistan, Andijan region, Andijan city, st. Soy 5, consent to the processing of my personal data, namely:

• Full Name;
• date of birth;
• address of actual residence;
• address of registration;
• contact phone number;
• e-mail address;
• information about education (level of education, country, city, name of the institution that issued the diploma of education, faculty, specialty, period of study, academic degree);
• information about additional education (name of the course, organization, document number, form of study, period of study);
• information about work experience for the last 10 years (organization, city, subdivision, position, information about the recommending person (position, contact phone number), period of work);
• military service duty status;
• additional information (computer skills, knowledge of professional software, marital status, driver's license, language proficiency, information about criminal prosecution);
• other information that may be contained in the CV submitted by me.

The purpose of processing personal data is to assist in finding a vacant position and possible further employment. This consent is provided for the following actions (operations) with my personal data: collection, recording, systematization, accumulation, storage, clarification (updating, changing), extraction, use, transfer (provision, access), deletion, destruction, carried out as with using automation tools (automated processing), and without the use of such tools (manual processing). In the event of a change in my personal data, I undertake to inform JV ANDIJANPETRO LLC in writing and submit copies of supporting documents. The consent comes into force from the date of its signing for the period of making a decision on the employment of the applicant for a vacant position, but not more than 1 year after receiving this consent. Consent may be withdrawn by me at any time on the basis of my written statement.
